[Quinn’s Power Edge Hockey Inc. “PEP Hockey”] – Off-Ice Hockey Training

Release of Liability, Assumption of Risks, and Agreement to Return Product

I, the undersigned, acknowledge that I am voluntarily participating in off-ice hockey training activities and utilizing training products provided by [PEP Hockey] (“the Company”). In consideration for being permitted to use these training products, I agree to the following terms:

1. Assumption of Risks: I understand that participating in off-ice hockey training activities involves inherent risks and hazards, including but not limited to physical exertion, potential falls, and the use of training equipment. I acknowledge that these activities carry the risk of injury, and I voluntarily assume all such risks.

2. Release of Liability: I hereby release and discharge the Company, its owners, officers, employees, and agents from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during the course of off-ice hockey training activities.

3. Product Ownership and Return Upon Request: I understand and acknowledge that I do not own the off-ice hockey training product provided by the Company. In the event that the Company requests the return of the product for any reason, I agree to promptly return the product in its original condition and packaging.

4. Company’s Right to Request Return: I acknowledge that the Company reserves the right to request the return of the training product at any time, or at the end of Power Edge Pro training programs and I agree to comply with such requests promptly.

5. Compliance with Instructions: I agree to use the off-ice hockey training product in accordance with the provided instructions, guidelines, and safety recommendations. I will seek assistance or clarification if I have any questions about the proper use of the product.

6. Medical Clearance: I hereby declare that I am in good health and have no medical conditions that would prevent me from safely participating in off-ice hockey training activities. I acknowledge the importance of consulting with a healthcare professional before engaging in any physical fitness program.

I have read and understand this Liability Waiver and Product Return Agreement. I voluntarily agree to its terms, and I am aware that by signing this document, I am giving up substantial legal rights.
